The information on this web site (“Website”) has been created for your hospital by nThrive, Inc., (“nThrive”).
nThrive recognizes the importance of your privacy. We understand that your hospital bill includes very personal information. This Privacy Policy and Agreement ("Privacy Policy") will tell you what information we collect and how it is used. We urge you to read this Privacy Policy carefully so that you will understand clearly both our commitment to you and your privacy, and how we may collect and use information. By using the Website site, you agree to the terms of this Privacy Policy. The policy may change from time to time so please check back periodically. If you have any questions about our privacy policies, we encourage you to contact us using any of the methods outlined at the end of our Privacy Statement.
nThrive is a licensee of the TRUSTe Privacy Program. TRUSTe is an independent, nonprofit organization whose goal is to build users' trust and confidence in the Internet by promoting the use of fair information practices. As required of TRUSTe licensees, we will inform you of the following in this privacy policy:

Information Collection
Personally Identifiable Information (PII or Personal Information) is information that can be used to identify or contact you. When you visit the Website, MD-X collects only the Personal Information you provide to us when providing/updating your user information or paying your hospital bill. Examples of information collected may include, but is not limited to: hospital account number, social security number, health plan and date of birth. Furthermore, users may provide contact information (such as name, email, and home address) and financial information (such as credit card number, expiration date). This information is used for billing purposes only.
Information Use
Profile
We may store information that we collect through cookies, log files, clear gifs, and/or third parties to create a profile of our users. A profile is stored information that we keep on individual users that details their viewing preferences. Consequently, collected information is tied to the users personally identifiable information to provide offers and improve the content of the site for the user. This profile is used to tailor a user’s visit to our Web site, and to direct pertinent marketing promotions to them. We do not share your profile with other third parties.
Cookies
A cookie is a piece of data stored on the user’s computer tied to information about the user. Usage of a cookie is in no way linked to any personally identifiable information while on our site. While we use session ID cookies on the website, we do not use persistent cookies. For the session ID cookie, once users close the browser, the cookie simply terminates. A persistent cookie is a small text file stored on the user’s hard drive for an extended period of time.
Third Party Advertising
At this time, nThrive does not allow any third-party advertising on the website. Should nThrive implement third-party advertising it will be noted in an updated and amended version of this Privacy Statement.
Log Files
Like most standard Web site servers we use log files. This includes internet protocol (IP) addresses, browser type, internet service provider (ISP), referring/exit pages, platform type, date/time stamp, and number of clicks to analyze trends, administer the site, track user’s movement in the aggregate, and gather broad demographic information for aggregate use. IP addresses, etc. are not linked to personally identifiable information.
Clear Gifs (Web Beacons/Web Bugs)
At this time, we do not employ a software technology called clear gifs (a.k.a. Web Beacons/Web Bugs), to manage content on our site.

Though we make every effort to preserve user privacy, we may need to disclose personal information when required by law wherein we have a good-faith belief that such action is necessary to comply with a current judicial proceeding, a court order or legal process served on our Web site.
Aggregate Information (non-personally identifiable)
We share aggregated demographic information with our partners and clients. As an example, we may provide our clients with aggregate information on the users of our site (users by zip code, county or other geographic or similar parameters). Hospital’s use of Third Party Intermediaries Your hospital may decide to use a credit card-processing firm and/or a mail service to bill you for services render. We will provide only the minimum required data set that enables these services to perform their required contractual duties.

Security
This Web site takes every precaution to protect our users’ information. When users submit sensitive information via the Web site, their information is protected both online and off-line.
When our registration/order form asks users to enter sensitive information (such as credit card number and/or social security number), that information is encrypted and is protected with the best encryption software in the industry - SSL. While on a secure page, such as our order form, the lock icon on the bottom of Web browsers such as Netscape Navigator and Microsoft Internet Explorer becomes locked, as opposed to un-locked, or open, when users are just ‘surfing’.
While we use SSL encryption to protect sensitive information online, we also do everything in our power to protect user-information off-line. All of our users’ information, not just the sensitive information mentioned above, is restricted in our offices. Only employees who need the information to perform a specific job (for example, our billing clerk or a customer service representative) are granted access to personally identifiable information. All employees are provided unique passwords that must be used each and every time they enter our database. Furthermore, ALL employees are kept up-to-date on our security and privacy practices. Every employee signs a confidentiality and privacy document at the time of hire and is continually reminded about the importance we place on privacy, and what they can do to ensure our users’ information is protected. Any employee who abuses these policies is subject to termination and legal action. Finally, the servers that store personally identifiable information are in a secure environment, in a locked facility with limited user access.
Although we will make every reasonable effort to protect your Personal Information from loss, misuse, or alteration by third parties, you should be aware that there is always some risk involved in transmitting information over the Internet.

In order for this Web site to properly fulfill its obligation to users it is necessary for us to supplement the information we receive with information from 3rd party sources.
Address and Health Plan Verification
We use third party software to verify a user’s address and health plan eligibility to ensure bills are sent to the proper entity and location.
Correcting/Updating/Deleting Personal Information
If a user’s personally identifiable information changes (such as zip code, phone, email or postal address) we provide a way to correct or update users’ personally identifiable information. This can usually be done at the update page.
Notification of Changes
If we decide to change our privacy policy, we will post those changes to this privacy statement, the homepage, and other places we deem appropriate so our users are always aware of what information we collect, how we use it, and under what circumstances, if any, we disclose it. We will use information in accordance with the privacy policy under which the information was collected.
If, however, we are going to use users’ personally identifiable information in a manner different from that stated at the time of collection we will notify users via email. Users will have a choice as to whether or not we use their information in this different manner. However, if users have opted out of all communication with the site, or deleted/deactivated their account, then they will not be contacted, nor will their personal information be used in this new manner.
